Registration Form:

We ask all parents/guardians to bring along completed registration forms to our coaching sessions. Click the link below to download the Primo Football Academy registration form. Don't worry if you do not have access to a printer as these forms will be available at all coaching sessions. However these forms will have to be completed before your child can begin training. Please note all our coaches are First Aid certified and hold up-to-date Disclosure Scotlands. These registration forms are for the childs safety and will be used to gain contact information incase of emergency.
